This form is designed to provide a means for the faculty to rate the professional performance and leadership characteristics of the administrator named above.

For items 1 - 5 indicate to what extent you agree that a specific statement is characteristic of this administrator. Please be certain to mark one choice for every statement in items 1 - 5.
